Situation
A nationwide healthcare provider is planning to implement a drug conversion from one drug therapy to another drug therapy across all of their US facilities and patients within a couple months
Challenge
Dosing units, formulations, frequency, and protocols of administration are different between the new and old therapy. Furthermore, dose conversion is not linear between the two therapies
The provider’s current automated dosing algorithm system is set up for the previous drug dosing units and cannot be updated quickly because of an outdated legacy system. Converting patients en masse increases the possibility for human error when calculating dose conversions
Ongoing dose adjustment process for the maintenance of the new therapy are also not develop nor are staff familiar or trained to handle, administer, store, and determine correct dose adjustment for the new therapy

Conducted analysis to generate a proprietary non-linear dosing conversion model between the new and old drug therapy accounting for change in dosing units, formulations, frequency, and protocols of administration and informed by scientific literature
Took over real-time individual patient conversion dose calculations by operationalizing a process to analyze each patient’s new lab values, calculate appropriate dose on the old therapy, convert old therapy dose to a new therapy dose, confirm medical accuracy and judgment, and communicate back out to each provider site with appropriate patient conversion dose and instructions
Developed an online app/tool as an aid for nurses and clinicians to provide point of care calculations of the appropriate dose adjustment as well as staff training sessions on new clinical operations related to the new drug therapy
